The UCSF Survivors of Childhood Cancer Program helps survivors of pediatric cancer achieve optimal physical and emotional health through medical care, education and research.
About one in 500 young adults in the United States is a survivor of pediatric cancer due, in part, to advancements in cancer treatments that have increased survival rates. Many new treatments, however, are now known to cause long-term side effects that occur years and even decades later.
If you are a survivor of childhood cancer, it is important to receive long-term, follow-up care by heath care professionals knowledgeable about your medical history.

The Survivors Clinic is held bi-weekly for survivors of pediatric cancer who have completed treatment at least two years ago. The clinic visit includes a consultation with a pediatric cancer specialist, nurse and social worker. A phone consultation with a school liaison may be scheduled upon request. The entire visit takes one or more hours and includes:
Prior to your clinic visit, we may request that you undergo certain tests, such as X-rays, blood work and other procedures that monitor your health. Our clinic coordinator will discuss these tests prior to your clinic visit and will assist you with any health insurance issues.
After visiting the Survivors Clinic, you'll receive a Survivor Health Passport, a wallet-sized card that includes information about your disease and treatment history, risk factors and follow-up recommendations based on the Children's Oncology Group (COG) guidelines. These guidelines can be viewed at www.survivorshipguidelines.org. The Survivor Health Passport should be carried by patients and shared with other health care providers to ensure the best possible follow-up care.
We recommend that you visit the Survivors Clinic at the following intervals — two years after completing treatment, five years after completing treatment and then once every five years. More frequent visits can be arranged as needed.
Our clinic coordinator can assist you with any health insurance matters, such as authorization for the clinic visit and pre-visit tests.
The Survivors Clinic is held in the afternoon of every other Wednesday of each month.
