Refer a Patient: Cardiopulmonary Exercise Lab

To refer a patient for a cardiac test or a cardiac monitor, follow these steps. If you have questions, please call us at (415) 476-3774.

1. Have your patient call 877-UC-CHILD:

Your patient can call 877-UC-CHILD (877-822-4453) now for an appointment. We can schedule the visit while you're preparing the referral – there’s no need to wait.

2. Gather required documents

You'll need copies of your patient's demographics, insurance authorization and clinic notes.

3. Fill out the referral form

Download and complete the Request for Cardiac Testing/Cardiac Monitor form.
