Do I need to contact the clinic before sending pre-adoption records?
We recommend calling or emailing just before sending the records, because there are times when we are out of the office. Otherwise, you are welcome to call, but it is not necessary to make a reservation.

What is the turn-around time?
Every effort is made to provide a quality assessment within two working days; oftentimes, one working day is sufficient. If there are special time constraints, please let us know.

What is included in the pre-adoption assessment?

  • A meeting (in person or by phone) up to 30 minutes in length
  • A written report of 1 to 2 pages in length
  • Appropriate growth charts

Additional information will be analyzed as it is received, up to the time of adoption. (This is particularly important for families adopting from Guatemala, but pertains to all countries.)

What information should be sent?
We would like to have as much information about the child as you can find. Please do not edit the material before sending it. Send records in their original language as well as an English translation.

The Pre-adoption Client Information and the Consent for Pre-adoption Evaluation will also need to be filled out.

How should the information be sent?
Email is a good way to send information, especially photos. Faxing is good for documents, but is not recommended for photos. US Mail, FedEx, or UPS may be used to send videotapes or photos. Please send copies only—never the originals—because we will not be able to return them. (Note: The consent form calls for signatures; if you are returning this by email, type in your name and the name of the witness.)

What if the material is not translated?
The adoption agency generally provides translation upon request or refers you to a translator. If you cannot arrange translation, let us know.

Does insurance cover pre-adoption record reviews?
No, because the child is not covered on your insurance until the day that she or he is legally adopted. Reviews do apply to the adoption tax credit.

How are payments made?
Credit card payment is preferred (VISA or MasterCard), but personal checks are fine. Generally, we ask for credit card information at the time of the phone conference. Checks are payable to Nancy Curtis, MD.
