If this is your first appointment at the clinic, please ask your child's pediatrician or specialist to fax or send the following information to (415) 353-2889:

• Written referral, describing your child's condition
• Your name, address and phone number, and your child's name
• Related medical records lab tests and imaging reports such as MRI scans, ultrasounds,
• If prior surgery, a pathology report
• Insurance information, including an "authorization" for the appointment if your insurance company requires it

After the information is submitted, our doctors will evaluate your child's reports and a representative will call you, typically within two days. Please ask your child's doctor to indicate if your appointment request is urgent.
