If this is your first appointment at the center, please ask your child's pediatrician or specialist to provide a written referral, if your health insurance requires it, and fax it to the clinic coordinator at (415) 514-2470. In addition, please fax or send the following information:

• All neurological medical records related to your child's condition
• All lab test results, such as for blood tests, and radiology reports for scans such as MRIs, CTs and ultrasounds, related to the condition
• Immunization records
• Copy of your insurance card (front and back) and an "authorization" for the visit if your health insurance requires it
• Completed new patient registration form and medical records release form

After our doctors evaluate your information, the clinic coordinator will contact you to discuss your appointment request. If you have any questions, please call the clinic coordinator at (415) 353-3939 from 9 a.m. to 5 p.m., Monday to Friday.

The clinic is open on Tuesday. Neurologists are available from 9 a.m. to noon. A neuro-psychologist is available from 8 to 11 a.m. and a neuro-ophthalmologist is available from 1 to 3 p.m.
