Most clinics at UCSF Benioff Children's Hospital require a referral from your child's pediatrician or specialist to make an appointment. You may download a referral form to send to your health care provider here.
The process of requesting an appointment varies from clinic to clinic. To get started, please select the clinic you'd like to visit below. Then look for the "Request an Appointment" section on the clinic's web page for phone numbers and instructions for making an appointment.

Patients who reside outside of the United States can contact International Services for help with appointments, accommodations, visa support and an estimate of fees.
